PerioMechanics

Notes

Journal

Notes from clinic. Written for colleagues. Readable if you have sat through one honest periodontal explanation.

No invented papers. No unpublished trial percentages. If you wanted plates first, the lab is the ordered path.

HR toggle covers the lab chrome and patient pages first; essays stay in the language they were written.

Force changes what is wrong and where

Force in a loaded, perfused periodontium changes the biochemical environment of the millimetre gap, that environment changes the signal, and the signal relocates what is wrong and where.

    What PerioMechanics is

PerioMechanics is first-principles periodontology. It treats the periodontium as a load-bearing, perfused organ — one interacting system of mechanics, perfusion, biochemistry, and signaling — and uses biomechanics, fluid dynamics, perfusion theory, and finite-element analysis to get into the depths of biology.

    The periodontium is an architecture

Gum, ligament, and bone are a load-bearing, perfused construction. The periodontal architect is the clinician who names walls, blood, and load before the kit.

    Mucogingival surgery is geometry

Root coverage is a tent, a wall, and a blood supply. Cairo's recession types are a wall count. Tension is a perfusion event wearing a knot.

    How an attachment lasts

An attachment lasts if the load, the blood, and the geometry still agree after years. That is a chair sentence, not a wellness slogan.

A flat ridge is not a cup

Vertical bone on a denuded root with no bony peaks is the hard problem: no container, no lateral blood, and epithelium always wins the race unless you tent and feed.

A tight suture is a vascular event

Blanching is not a vibe. It is edge pressure crossing capillary closing pressure.

The graft does not die of hypoxia. It dies of distance.

Fibroblasts tolerate days of low oxygen if glucose still reaches them. The killer is millimetres from a perfused face.

Look at the papilla first. Then count the walls.

Root coverage height is set by the interdental attachment you already have. Bone fill is set by the container you have left.

Perfusion is not a footnote: why PDL blood flow belongs in surgical simulation

We suture for blood supply. Then we simulate as if the ligament were a dry rubber. The contradiction would be funny if it were not in the treatment plan.

Peri-implantitis: if the foundational model is incomplete, how long can we rely on it?

Biofilm explanations of peri-implantitis are not false. They are unfinished. Unfinished models are how a field keeps repeating the same surgery with more confidence.

Clinical sketch → FEA: closing the gap between periodontology and MedTech engineering

We already think in load paths. We just hand the engineer a radiograph and a smile and hope the constitutive model arrives by intuition.
